Initial Visit

Once scheduled for Mohs surgery, you will be mailed a packet of information which includes directions, pamphlets about the procedure, and paperwork. Please fill this out prior to your appointment and bring it in with you. We hope that by providing this form to you in advance, your visit will be more efficient and more informative.
